Cancellations/SubstitutionsTO QUALIFY, ALL FORMS MUST BE RECEIVED TOGETHER BY OCTOBER 1. Additional forms received after the group registration has been processed are not eligible for the discount. Group discounts are no longer available after October 1 and do not apply to Slot Manager Institute, Exhibits Only or Keynote Speaker Session packages. To register for Group Discounts, please print this form and fax or mail with payment as indicated on the form.
Cancellations must be received in writing by September 28 and are subject to a $100.00 processing fee. After September 28, registration fees are non-refundable. You may substitute another person at anytime by submitting written authorization.
